dc.description.abstract Aim. To assess the effect of PRP therapy on ovarian function and folliculogenesis in women of late reproductive age with low ovarian reserve. Materialsand Methods. The study included 140 women aged 36 to 43 years who presented with infertility and reduced ovarian reserve. The patients were divided into three groups: Group I included 60 women with low ovarian reserve (AMH < 1.0 ng/ml, AFC < 5), Group II included 50 women with very low ovarian reserve (AMH < 0.5 ng/ml, AFC < 3), Control group included 30 healthy women with normal ovarian reserve (AMH > 1.5 ng/ml, AFC > 5). Results. The study showed that PRP therapy significantly improved ovarian reserve in women of late reproductive age with low ovarian reserve. AMH levels increased by 41%, and FSH levels decreased by 25% in Group I. In the group with very low ovarian reserve, positive changes were also noted. ROC analysis demonstrated a high predictive value for AMH and antral follicle count markers (AUC = 0.88 for Group I and 0.80 for Group II) for predicting successful pregnancy following PRP therapy. Conclusion. PRP therapy is a promising method for improving ovarian function and folliculogenesis in women with low ovarian reserve. Following therapy, AMH levels increased by 41%, and FSH levels decreased by 25%, enhancing the likelihood of a successful pregnancy. High AUC values obtained through ROC analysis (0.88 for Group I and 0.80 for Group II) confirm that changes in AMH levels and antral follicle count are reliable markers for predicting the success of therapy. en_US
